How
to Donate
Medical Volunteers:
Because
we currently work as volunteers of the Gardner
Family Health Network, the credentialing process
is the same as it would be for their paid staff.
Gardner provides malpractice insurance for this
volunteer work. The following paperwork is required
for credentialing:
- Copy
of medical license
- Proof
of current CPR (BLS)
- Current
DEA license
- Physical
dated within the last 6 months
- PPD/TB
screening within the last 6 months
- Record
of immunizations and antibody titers (e.g.,
hepatitis B, varicella)
- Copy
of current CV
- Copy
of two forms of ID (e.g., driver's license,
passport, Social Security card) for W-2 form
- Completion
of application packet from Human Resources department
(We will provide this to you.)

Interested
nurse practitioners and physician assistants can
complete the above process, in addition to filling
out additional forms pertaining to medical supervision.
(These forms will also be provided.)

We
work as volunteer dentists at SVCHA, independent
of CityTeam Ministries and the Gardner Family
Health Network. SVCHA does not provide malpractice
insurance, and the volunteer dentist must make
sure that he/she has a malpractice insurance carrier.

- Proof
of current CPR (BLS)
- Current

screening within the last 6 months

of immunizations and antibody titers (e.g.,

Interested
dental volunteers should contact Dr. Allen Vu.
Prayer
ministers are an integral part of our ministry.
Since we are currently located on site at City
Team, which is an evangelical Christian organization,
the atmosphere is very open to prayer and spiritual
counsel. Spiritual care is a deep need in our
patient population. Some of our patients already
have faith in Jesus while others are seeking encouragement
from other believers. Some do not believe in Jesus
but are desperate for any kind of help. Ministering
to the underserved is a great way to experience
God.
The
role of a prayer minister is to help patients
get connected to God by lifting them up in prayer.
Prior prayer ministry experience, though helpful,
is not mandatory. Training is provided by SVCHA.

Volunteer
orientations are offered to all medical, dental,
and prayer volunteers. Attendance at this orientation
is mandatory in order to start volunteering. Topics
such as SVCHA by-laws, values, policies, and safety
are discussed. We reserve the right to screen
all volunteers. Contact Lorri Tsai or Linh Ly
to attend an orientation.
